Tennessee Licensed Care Coordinator

Job Summary

To provide comprehensive care coordination for Medicaid and Medicare members, the full-time Tennessee Licensed Care Coordinator will build trusted relationships, conduct assessments, and facilitate access to necessary healthcare services while working remotely from Tennessee.

Key Responsibilities

• Conduct outreach to educate and motivate members about program benefits
• Assess member status and develop individualized care plans to meet their goals
• Manage a caseload to ensure timely communication and coordination of care with healthcare providers

Required Qualifications

• Active and unrestricted LPN or RN license in Tennessee
• Minimum of 2 years of experience in care coordination, case management, or related fields
• Fluency in English; Spanish fluency preferred
• 1+ years of remote work experience
• Experience with Medicaid and/or Dual Eligible patients
