Job Summary
Providing intervention and managing client cases, the full-time Tennessee Licensed Care Coordinator will serve as a resource link for children and families in need, working collaboratively with treatment teams to develop effective plans and advocate for struggling individuals.
Key responsibilities
- Manage client cases and provide intervention services to children and families
- Collaborate with treatment teams to develop and evaluate care plans
- Connect clients to resources and organizations that enhance their well-being
Required qualifications
- Bachelor's degree in a Human Service discipline from an accredited institution
- CPR and First Aid certified, or willingness to obtain certification
- Experience working with children and adolescents in a therapeutic, community-based setting
- Ability to work flexible hours and across multiple locations
