Job Summary

A company is looking for a Care Management Pharmacist.

Key Responsibilities

• Establish and manage pharmaceutical therapies for patients living with kidney disease
• Consult on medication therapy management and monitor drug therapy for optimal outcomes
• Educate patients and caregivers on medications and chronic disease management

Required Qualifications

• Doctorate or bachelor's degree in pharmacy
• Unrestricted Pharmacy state license in TN, AL, or CA, or ability to acquire one within 60 days
• Strong understanding of pharmaceutical therapies focused on patient quality of life
• Comfortable delivering educational seminars on pharmaceutical topics
• Intermediate skills with MS Office Suite and ability to work remotely
