Job Summary

A company is looking for a Care Manager (RN) to support their Ambetter Marketplace members remotely.

Key Responsibilities

Evaluate member needs and barriers to care, recommending plans for optimal outcomes
Develop and collaborate on ongoing care plans, ensuring access to necessary resources and services
Monitor member progress and revise care plans as needed, providing education and support to members and families

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience in case management, home health, or managed care
Must hold a valid RN license, including compact state licensure
Residency in North Carolina, Tennessee, or Georgia, working in the Eastern Time Zone
