Job Summary

A company is looking for a Case/Care Manager.

Key Responsibilities

• Conduct assessments and develop tailored care plans for clients
• Coordinate care and resources to support clients in achieving their goals
• Monitor client progress and ensure compliance with program requirements

Required Qualifications

• Degree in social work, RN, or LPN preferred
• Experience with community resources related to social determinants of health
• Ability to work remotely with high-speed internet access and in a private environment
• Proficiency in Microsoft Word, Excel, and Outlook; must be able to learn new software
• Ability to pass a Criminal Background Check, Drug Screen, and DMV Check
