Job Summary

A company is looking for a Case Manager - LPN.

Key Responsibilities

• Initiate referrals and support care coordination workflows by gathering necessary clinical and social information
• Perform structured case management support activities including follow-up calls and resource coordination
• Document all case management activities accurately and communicate with members and providers regarding case status

Required Qualifications

• Licensed Practical Nurse (LPN) with an active license in Tennessee or a participating Nurse Licensure Compact state
• 1-2 years of clinical experience required
• Knowledge of community resources and service authorization processes
• Familiarity with care management frameworks and regulatory requirements
• Digital literacy assessment may be required
