Job Summary
To enhance patient care through effective management, the full-time Tennessee Licensed Clinical Manager will oversee clinical support staff, develop care guidelines, and manage chronic and transitional care initiatives in a remote setting.
Key responsibilities
- Collaborate with clinical leaders to develop care guidelines and improve patient interactions
- Provide leadership and guidance to clinical support staff, ensuring high-quality care delivery
- Manage chronic and transitional care initiatives, monitoring effectiveness and implementing improvements
Required qualifications
- Active Registered Nurse (RN) license or other relevant clinical licensure
- 3-5 years of clinical experience in a healthcare setting, with 2-3 years in a leadership role
- Experience with chronic care management and transitional care management
- Familiarity with telehealth care delivery and home-based patient care
- Proficiency in healthcare software and Electronic Health Records (EHR) systems
