Job Summary

A company is looking for a Clinical Outreach Specialist - LPN.

Key Responsibilities

• Conduct targeted outbound outreach to members with positive screening or diagnostic results
• Apply clinical judgment to explain test results and the importance of timely follow-up
• Document outreach activities and assist with navigating barriers in collaboration with internal teams

Required Qualifications

• Active, unrestricted Licensed Practical Nurse (LPN) license
• 2 years of experience in patient/member education, clinical follow-up, or care coordination
• Knowledgeable of HEDIS measures and health plan standards
• Familiarity with preventive screening workflows
• Comfortable working with sensitive health information
