Tennessee Licensed Discharge Care Nurse

Job Summary

To support high-risk members, the remote Tennessee Licensed Discharge Care Nurse will implement telephonic case management interventions, assess discharge needs, and collaborate with clinical teams while working full-time.

Key responsibilities

• Evaluate member discharge needs and develop readmission prevention plans
• Coordinate care post-discharge by collaborating with providers and members
• Document discharge activities and participate in team meetings and education discussions

Required qualifications

• Bachelor of Science in Nursing
• Current, unrestricted Compact RN license in the State of Tennessee
• 5+ years of recent experience in the acute inpatient hospital setting
• 2+ years of experience in discharge planning and/or case management
• Experience with multiple health insurance products, including regulatory and compliance requirements
