Key Responsibilities

• Implement coaching techniques and behavior change solutions to improve patient outcomes
• Provide video consultations and support patient care plans using motivational interviewing and SMART goals
• Collaborate with interdisciplinary teams to ensure optimal patient-centered care and address any gaps in care

Required Qualifications

• Board Certified Health and Wellness Coach
• 2+ years of experience as a health coach or wellness educator
• Proficiency in virtual care and database management
• Knowledge of motivational interviewing and behavioral change techniques
• University degree in health science or social science discipline (preferred)
