Job Summary

A company is looking for an Afterhours Triage Registered Nurse Hospice.

Key Responsibilities

• Assess physical, environmental, and emotional factors telephonically to determine hospice needs
• Utilize EHR to develop recommendations based on the patient's plan of care
• Collaborate with pharmacies and other agencies for effective patient management and assign visits to on-call field staff

Required Qualifications, Training, and Education

• Current, unencumbered license to practice as a registered nurse in the assigned state
• One year of experience as a registered nurse
• Current CPR certification
• Knowledge of the needs of terminally ill patients and their caregivers
• Must be willing and eligible to obtain additional RN licenses in other states
