Job Summary

To provide remote telehealth support for Medicare beneficiaries, the full-time Tennessee Licensed Nurse Practitioner will conduct evaluations for urgent clinical concerns, collaborate with healthcare teams, and promote patient education while working from home.

Key responsibilities

• Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent, non-emergent clinical concerns
• Determine appropriate interventions and escalate care when necessary
• Provide patient education on disease management and promote medication adherence

Required qualifications

• Active, unrestricted Nurse Practitioner license
• National certification appropriate to specialty
• Minimum of 3 years of clinical practice experience in relevant fields
• Experience managing hypertension and common chronic conditions
• Prior telehealth or virtual care experience
