Job Summary

A company is looking for an RN-Recovery Care Coordinator (Remote).

Key Responsibilities

• Facilitates communication and coordination among care team members
• Acts as a patient advocate and navigator throughout the care model
• Monitors patient care and collaborates with providers to ensure access to necessary services

Required Qualifications

• Associate of Science in nursing
• Current RN license specific to the assigned state
• Three to five years of acute RN experience
• Preferred: Bachelor of Science in nursing
• Preferred: Care Coordination and Transition Management (C.C.C.T.M) certification
