Job Summary

A company is looking for a Recovery Partner (REMOTE) to provide virtual care support.

Key Responsibilities

• Lead communication and coordination of care among the care team members
• Support recovery care coordination and assist in monitoring patient progress
• Coach patients in self-management and facilitate closure of care gaps

Required Qualifications

• High school diploma or equivalent
• LPN, EMT A, or Paramedic certification required
• Willingness to learn motivational interviewing techniques
• Willingness to learn a self-management model for patient support
• Preferred EMT-B certification or relevant medical professional experience
