Job Summary

A company is looking for a Multi-State Remote Registered Dietitian, CDCES (Contracted).

Key Responsibilities

• Provide call-based care and clinical nutrition management to patients in the Remote Patient Monitoring (RPM) program
• Assess member health regimens and interpret blood glucose and CGM readings related to nutrition
• Craft individualized meal plans based on member health needs and collaborate with the care team to support clinical goals

Qualifications

• Education: BS in Nutrition
• License: Valid, unencumbered state RD license, with preference for TN, OH, MS, LA
• Certification: CDR and CDCES required
• Experience: 3+ years managing patients with diabetes preferred
• Knowledge: Familiarity with continuous glucose monitors (CGMs) is a plus
