Tennessee Licensed RN Recovery Coordinator

Job Summary

A company is looking for an RN-Recovery Care Coordinator (REMOTE).

Key Responsibilities

• Assist patients as a patient advocate and provide after-hours triage support
• Monitor alerts from patients' homes and escalate changes in condition to providers
• Generate and present operational, clinical, and quality reports while managing workflows related to patient needs

Required Qualifications

• Associate of Science in Nursing
• Current, active RN licensure in good standing, with willingness to obtain multi-state licensure
• Three to five years of acute nursing experience, including two years of telephonic triage experience
• General knowledge of clinical standards and outcome measurement
