Job Summary

To support a growing practice, the PRN Social Worker will provide high-quality therapeutic services to older adult patients in a fully remote environment, collaborating with an interdisciplinary team and participating in community education initiatives.

Key responsibilities

• Collaborate with a small interdisciplinary team to deliver therapeutic services, including group, individual, and family therapy sessions
• Participate in community education efforts to promote the importance of behavioral health among local medical facilities and community groups
• Facilitate treatment team meetings and ensure compliant documentation and patient-centered treatment plans are developed and updated

Required qualifications

• Tennessee Licensed Social Worker or Counselor (LISW, LCSW, LPC, LMFT, etc.) - required
• Commitment to working with kindness and accountability to enhance patient quality of life - required
