Job Summary
Seeking a Tennessee Licensed Therapist (LCSW, LPC, LMFT) for a full-time remote position in the Substance Use Disorder Program, where the therapist will assess, plan, and implement individualized care strategies while conducting individual and group therapy for substance-exposed members.
Key responsibilities:
- Conduct biopsychosocial assessments and develop individualized care plans in collaboration with members, families, and healthcare providers
- Provide individual counseling and group therapy, while identifying referrals for social service programs as needed
- Document member assessments and care plans, and participate in interdisciplinary team meetings to ensure effective transitions of care
Required qualifications:
- Active LCSW, LPC, or LMFT license in Tennessee
- Preferred experience working with substance abuse populations
- Background in maternal and substance abuse is preferred
- Experience working with both adults and adolescents
- Familiarity with Medicare and Medicaid procedures
