Job Summary
Seeking a full-time Tennessee Licensed Therapist (LCSW, LPC, LMFT) to join the Substance Use Disorder Program remotely, where the therapist will assess, plan, and implement individualized care strategies while providing individual and group therapy to substance-exposed members.
Key responsibilities:
- Conduct biopsychosocial assessments and develop individualized care plans in collaboration with members and their families
- Provide individual counseling and group therapy for adolescents and adults, while advocating for members' needs within the healthcare team
- Document assessments, care plans, and referrals, and participate in interdisciplinary team meetings to ensure effective transitions of care
Required qualifications:
- Valid LCSW, LPC, or LMFT license in Tennessee
- Preferred experience working with substance abuse populations
- Background in maternal substance abuse is preferred
- Experience working with both adults and adolescents
- Familiarity with Medicare and Medicaid procedures
