Job Summary
Guiding members through transitions of care, the full-time Tennessee Licensed Transition of Care Nurse will provide remote support for discharge planning and post-acute transition needs, ensuring comprehensive and safe care coordination.
Key responsibilities
- Provide telephonic or onsite services to assist hospital care coordination teams with discharge planning
- Assess resource utilization, develop treatment plans, and establish measurable goals for transitioning members to appropriate care levels
- Evaluate clinical status and overall resource utilization to promote quality and cost-effective outcomes
Required qualifications
- Active Registered Nurse (RN) license in Tennessee or a participating Nurse Licensure Compact state
- Minimum of 3 years of clinical experience
- At least 5 years of experience in the healthcare industry
- Strong background in ICU, Transition of Care, Skilled Nursing Facilities, Long-Term Acute Care Hospitals, or Case Management is preferred
- Ability to communicate effectively via digital channels and pass Windows navigation tests
