Job Summary

A company is looking for a Transitions of Care RN- 100% Virtual.

Key Responsibilities

• Conducts behavioral or clinical assessments to identify individual member needs
• Implements and coordinates health coaching plans and facilitates referrals to health professionals
• Monitors and evaluates the effectiveness of interventions and modifies plans as needed

Required Qualifications

• Associate's degree in nursing
• Minimum of 2 years of condition-specific clinical or home health/discharge planning experience
• Current unrestricted RN license in applicable state(s)
• Bilingual candidates are strongly preferred
• Experience in care of adult and chronically ill patients is preferred
