Tennessee Licensed Womens Healthcare Case Manager

Job Summary

A company is looking for a Women's Healthcare Case Manager (Remote).

Key Responsibilities

• Assess member needs and create individualized care plans
• Educate and support members through complex health transitions
• Coordinate care with healthcare providers and monitor progress

Required Qualifications

• Active Registered Nurse (RN) license in Tennessee or participating Nurse Licensure Compact state
• 3 years of clinical experience and 5 years in the healthcare industry
• Certified Case Manager (CCM) credential or must obtain within 2 years of hire
• Experience in women's healthcare (maternity, GYN, women's health) preferred
• Digital literacy and comfort in a remote work environment
