Job Summary

A company is looking for a Clerical Territory Patient Admissions Coordinator.

Key Responsibilities
  • Collaborates with referral sources, sales teams, and clients to gather information for quality referrals
  • Documents referral-related information and actions in the medical record accurately and timely
  • Ensures timely processing of referrals from various platforms and follows up with clients and caregivers as needed
Required Qualifications
  • High School diploma or GED
  • Minimum of one year experience in home care, hospice, or personal care services
  • Prior experience with Electronic Medical Records (EMR), HCHB preferred
  • Experience dealing with various healthcare payors, such as Medicare and Medicaid, preferred
