Job Summary

Key Responsibilities
- Collaborates with referral sources, sales teams, and clients to gather information for quality referrals
- Documents referral-related information and actions in the medical record accurately and timely
- Ensures prompt attention to referrals from various platforms and conducts follow-up calls to confirm information
Required Qualifications
- High School diploma or GED
- Minimum of one year experience in home care, hospice, or personal care services
- Prior experience with Electronic Medical Records (EMR), HCHB preferred
- Experience with various healthcare payors, including Medicare, Medicaid, and commercial payers, preferred
