Job Summary
To provide essential after-hours telephonic care, the part-time Nurse Practitioner licensed in Texas and Louisiana will manage high call volumes, deliver virtual assessments, and coordinate care for aging residents across multiple states while working remotely.
Key responsibilities
- Serve as the first line of support for residents and facility staff after-hours, providing direction and medical care over the phone
- Utilize Curana's telephonic platform to coordinate care between facilities, hospitals, and clinics
- Perform comprehensive assessments and document encounters accurately in the EMR, ensuring compliance with CMS requirements
Required qualifications
- Master's Degree as a Nurse Practitioner
- Active, unrestricted licensure in Texas and Louisiana
- Nurse Practitioner national certification as ANP, FNP, or GNP
- 3+ years of experience as a Nurse Practitioner preferred
- Background in acute and chronic disease management
