Job Summary
A company is looking for an Advanced Practice Provider NP/PA - Transitions of Care.
Key Responsibilities
- Perform initial transitions of care visits, including comprehensive assessments and medication reconciliation
- Conduct acute symptom transitions of care visits, diagnosing and managing patient needs
- Participate in interdisciplinary rounds to discuss high-risk patients and enhance team culture
Required Qualifications, Training, and Education
- At least 3 years of experience in a hospital, post-hospital, or primary care setting
- Ability to work full-time, including one weekend shift per month
- Active RN and APRN licenses or Physician Assistant licenses in good standing
- Active BLS certification at the time of hire
- Experience in internal medicine, urgent care, or geriatrics is preferred
