Job Summary

A company is looking for an Advanced Practice Provider NP/PA - Transitions of Care.

Key Responsibilities
  • Perform initial transitions of care visits, including comprehensive assessments and medication reconciliation
  • Conduct acute symptom transitions of care visits, diagnosing and managing patient needs
  • Participate in interdisciplinary rounds to discuss high-risk patients and enhance team culture
Required Qualifications, Training, and Education
  • At least 3 years of experience in a hospital, post-hospital, or primary care setting
  • Ability to work full-time, including one weekend shift per month
  • Active RN and APRN licenses or Physician Assistant licenses in good standing
  • Active BLS certification at the time of hire
  • Experience in internal medicine, urgent care, or geriatrics is preferred
