Job Summary
To provide transitions in care for patients and families post-visit, the full-time Texas Licensed Advanced Practice Provider will manage telephonic and online communications, addressing clinical alerts and ensuring quality patient care while working primarily onsite during evenings and weekends.
Key responsibilities:
- Respond to clinical alerts and manage care transitions through effective communication with patients and families
- Conduct assessments and provide health education, ensuring understanding of care instructions and resources
- Participate in quality assurance processes and contribute to team activities to enhance patient outcomes
Required qualifications:
- Master's Degree in Nursing or Doctor of Nursing Practice, or equivalent qualifications for Physician Assistants
- Three years of oncology experience in a hospital or clinic setting
- Active Texas Board of Nursing authorization as an Advanced Practice Registered Nurse or active PA certification
- Proficiency in Microsoft Word and experience with data entry and research
- Preferred bilingual proficiency in Spanish and English
