Job Summary
To support oncology patients in a virtual care environment, the full-time salaried Texas Licensed Advanced Practice Provider, Lead Care Manager will deliver high-quality transitions of care through phone and digital communication, demonstrating strong clinical judgment and exceptional communication skills while working evenings and weekends.
Key responsibilities
- Manage transitions of care by responding to clinical alerts and providing support for medication questions, care instructions, and follow-up appointments
- Conduct proactive assessments to prevent complications and close care gaps using oncology expertise and available resources
- Document all interactions in clinical systems and participate in quality assurance efforts to enhance service delivery
Required qualifications
- Current Texas Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) license
- Certification by the National Commission of Certification of Physician Assistants for PAs
- Basic Life Support (BLS) certification; Pediatric Advanced Life Support (PALS) may be required for specific departments
- Experience in oncology care and proficiency in motivational interviewing
- Ability to work autonomously while managing complex patient needs
