Job Summary
To deliver comprehensive, member-centered care coordination, the full-time Texas Licensed Bilingual RN Coordinator will engage with members proactively and reactively, develop individualized care plans, and educate both members and providers in a fully remote, technology-driven environment.
Key responsibilities
- Conduct outreach based on risk stratification and care gap data, utilizing motivational interviewing to enhance member engagement
- Develop and monitor individualized care plans through comprehensive clinical, psychosocial, and SDOH assessments
- Educate members and providers on benefits and digital health solutions, translating complex information into actionable guidance
Required qualifications
- Diploma from an accredited school of nursing required; ADN or higher preferred
- Current, unrestricted Texas RN license or active Compact (NLC) nursing license required
- 3-5 years of nursing experience in a direct patient care setting, with 1-2 years in a health plan or managed care organization
- Working knowledge of health plan benefits, CPT/ICD-10 codes, authorizations, and pre-certification processes
- Bilingual English/Spanish fluency required
