Job Summary
To support a growing care navigation program, the full-time Remote Texas Licensed Care Concierge will manage a caseload of 75-150 patients, providing proactive care coordination, education, and advocacy for older adults with serious chronic conditions while working remotely.
Key Responsibilities
- Serve as the primary point of contact for enrolled patients, conducting regular check-ins to assess health status and care needs
- Navigate patients through the healthcare system by coordinating appointments and facilitating communication between providers
- Conduct structured screenings for social determinants of health and connect patients with appropriate community resources
Required Qualifications
- Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential
- 1+ years of experience in remote care coordination or similar patient-facing roles
- Experience working with Medicare-enrolled or dual-eligible populations
- Strong understanding of care coordination principles and healthcare systems
- High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred
