Job Summary

To support a growing care navigation program, the full-time Remote Texas Licensed Care Concierge will manage a caseload of 75-150 patients, providing proactive care coordination, education, and advocacy for older adults with serious chronic conditions while working remotely.

Key Responsibilities
  • Serve as the primary point of contact for enrolled patients, conducting regular check-ins to assess health status and care needs
  • Navigate patients through the healthcare system by coordinating appointments and facilitating communication between providers
  • Conduct structured screenings for social determinants of health and connect patients with appropriate community resources
Required Qualifications
  • Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential
  • 1+ years of experience in remote care coordination or similar patient-facing roles
  • Experience working with Medicare-enrolled or dual-eligible populations
  • Strong understanding of care coordination principles and healthcare systems
  • High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred
