Texas Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator to enhance primary care delivery and patient support.

Key Responsibilities

• Engage high-risk patients to ensure follow-up appointments and close care gaps
• Collaborate with care team members and external vendors to support patient care
• Request medical records to ensure continuity of care for patients

Required Qualifications

• 2+ years of experience as a Licensed Practical Nurse (LPN), Certified Medical Assistant (MA), or Community Health Worker (CHW) preferred
• Certified Medical Assistant (CMA) or Community Health Worker (CHW) required
• Comfort with technology, including Microsoft Office products
• Prior experience with electronic health records for data management
