Texas Licensed Care Coordinator

Job Summary

To support patient care initiatives, the full-time Texas Licensed Care Coordinator will manage a panel of 400-450 patients, providing health coaching and conducting outreach while working remotely from a HIPAA-compliant home office.

Key responsibilities

• Conduct 50-60 daily outbound calls to patients for care plan support and health coaching
• Collaborate with clinical teams and caregivers to address patient needs and close care gaps
• Document all patient interactions in real time while maintaining HIPAA compliance

Required qualifications

• Must hold a nationally recognized medical assistant certification or be a Certified Pharmacy Technician
• Minimum 2 years of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability
• Must be legally authorized to work in the United States
