Job Summary
Supporting interdisciplinary care teams, the remote Texas Licensed Care Coordinator will facilitate scheduling, manage patient communications, and ensure seamless collaboration with health plan clinicians in a part-time capacity.
Key responsibilities
- Support scheduling for Complex Care Navigators and make patient reminder calls
- Facilitate collaboration with health plan care managers and ensure proper documentation of all patient contacts
- Answer patient calls, transfer escalated issues, and identify necessary patient documentation from patients and community providers
Required qualifications
- High School Diploma / GED (or higher)
- Experience with escalated customer service issue resolution
- Knowledge of Microsoft Office products and proficiency in Microsoft Excel
- Phone/Fax system experience
- EMR experience
