Job Summary

To support a growing care coordination team, the full-time Remote Care Coordinator Nurse will provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible members, ensuring they access appropriate care while working from a dedicated home office in Texas or select states.

Key Responsibilities
  • Conduct outreach to educate members about program benefits and facilitate their engagement in care
  • Assess members' needs and develop personalized care plans to address their health goals
  • Manage a caseload, coordinating care with providers and ensuring timely communication and documentation
Required Qualifications
  • Active and unrestricted LVN/LPN or RN license
  • 2+ years of experience as a care coordinator, case manager, or related role
  • 1+ years of remote work experience
  • Experience working with Medicaid and/or Dual Eligible patients
  • Must reside in Texas or select states including Louisiana, Mississippi, Arkansas, Missouri, Oklahoma, Kansas, Utah, Nebraska, or Wyoming
