Job Summary

A company is looking for a Care Coordination - RN.

Key Responsibilities

• Communicate and educate members and providers on insurance plan benefits and digital health solutions
• Provide pro-active and reactive support for members through various outreach methods
• Track and document work accurately using internal software tools and consult with supervisors as needed

Required Qualifications

• 1-2 years of experience working at a health plan, preferably with a commercial population
• 3-5 years of nursing experience in a clinical setting
• Current, unrestricted nursing license or Compact License
• Diploma from an accredited school/college of nursing
• Working knowledge of medical and insurance industry terminology
