Texas Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transitions of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare teams to support member transitions
• Facilitate communication and services between members, providers, and resources to meet health and socio-economic needs

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
• 2 - 4 years of related experience in care management
• Active RN license is required
• Experience in assessing health status and developing treatment plans
• Knowledge of compliance with state, federal, and third-party payer regulations
