Job Summary
To support a growing Population Health team, the full-time Texas Licensed Care Manager I - LVN will coordinate patient care, facilitate transitions, and address barriers to health while working remotely with occasional in-person meetings in Houston.
Key responsibilities
- Identify and arrange community services based on knowledge of Medicare, Medicaid, and other programs
- Coordinate patient care through assessment, planning, and implementation while advocating for patients' insurance and financial needs
- Maintain comprehensive clinical records and collaborate with interdisciplinary teams to ensure high-quality care and continuity
Required qualifications
- Active LVN license in the State of Texas
- Minimum of three years of clinical nursing experience, preferably in case management or care coordination
- Experience in hospital, acute care, or inpatient settings strongly preferred
- Knowledge of Medicare, Medicaid, and community-based resources
- Proficiency with electronic health records (EHRs) and care management systems
