Job Summary
Providing transitions of care through phone and online communication, the full-time Texas Licensed Care Manager will assist patients, families, and caregivers in navigating post-discharge needs, addressing medication questions, and improving overall patient outcomes while working remotely with a varying schedule that includes after-hours and weekend coverage.
Key responsibilities
- Respond to clinical alerts and patient inquiries via telephone and online communication channels
- Conduct proactive assessments to prevent complications and improve patient outcomes
- Accurately document all interactions and patient care activities in the electronic medical record
Required qualifications
- Bachelor of Science in Nursing (BSN) from an accredited school of professional nursing
- Current State of Texas Professional Nursing License (RN)
- Three years of oncology experience in a hospital setting
- Experience speaking with patients and families by phone
- Proficiency in Microsoft Word and internet research
