Job Summary
A company is looking for a Care Manager RN to develop and facilitate care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend care plans to improve healthcare outcomes
- Coordinate with providers and community resources to address unmet needs
- Monitor member status and update care plans as necessary
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
