Texas Licensed Case Manager Nurse

Job Summary

A company is looking for a Case Manager Registered Nurse (Remote, Texas).

Key Responsibilities

• Facilitate delivery of healthcare information and determine eligibility for benefits
• Develop and implement health strategies and programs to promote member wellness
• Coordinate care through assessment, planning, and advocacy for members' health needs

Required Qualifications

• Unrestricted and active TX/Compact RN licensure
• Minimum 3+ years of clinical practical experience, preferably with Medicare members
• Minimum 2+ years of case management, discharge planning, or home health care coordination experience
• Associates degree or equivalent experience required; Bachelor's degree preferred
