Texas Licensed Case Manager RN

Job Summary

To support a growing healthcare team, the full-time Texas Licensed Case Manager RN will assess, plan, implement, and coordinate care for clients remotely, focusing on improving health outcomes and providing health education and support.

Key responsibilities:

• Assess and interpret customer needs to develop appropriate care plans
• Identify solutions to complex client issues and provide guidance to less experienced team members
• Evaluate the effectiveness of interventions and adjust care plans as necessary

Required qualifications:

• Current unrestricted RN licensure in Texas
• Undergraduate degree or equivalent experience
• Experience in case management or clinical operations
• Knowledge of relevant state and federal healthcare regulations
• Ability to engage with clients and motivate them towards achieving health goals
