Job Summary

A company is looking for a Clinical Care Navigator RN.

Key Responsibilities

• Act as the primary clinical point of contact for members with multi-morbidities and chronic conditions
• Utilize data insights to identify gaps in care and opportunities for intervention
• Conduct holistic clinical assessments to build individualized care plans

Required Qualifications

• Graduate of an accredited nursing program with an active RN license
• Minimum of 2 years in Case Management within a health plan or managed care setting
• Experience managing complex, high-acuity cases
• Comfortable in a Google Suite environment and quick to learn new technology platforms
• Preferred certifications include CCM or CMCN; BSN is preferred
