Job Summary

To improve health quality outcomes for UHC Medicare members, the part-time Texas Licensed Clinical Pharmacist will perform telephonic outreach, optimize pharmacy therapy, and oversee pharmacy projects while working remotely with occasional office visits in Houston, TX.

Key responsibilities

• Perform telephonic outreach to enhance medication adherence and appropriate drug utilization
• Engage with providers and pharmacies to optimize pharmacy therapy
• Document and track member engagement to improve CAHPs scores and member experience

Required qualifications

• Bachelor's degree (or higher) in Pharmacy
• Current and unrestricted Pharmacist license in Texas
• 3+ years of experience as a licensed pharmacist
• 3+ years of patient consultation experience
• Beginner level proficiency in MS Office and Windows-based environments
