Job Summary
To improve health quality outcomes for UHC Medicare members, the part-time Texas Licensed Clinical Pharmacist will perform telephonic outreach, optimize pharmacy therapy, and oversee pharmacy projects while working remotely with occasional office visits in Houston, TX.
Key responsibilities
- Perform telephonic outreach to enhance medication adherence and appropriate drug utilization
- Engage with providers and pharmacies to optimize pharmacy therapy
- Document and track member engagement to improve CAHPs scores and member experience
Required qualifications
- Bachelor's degree (or higher) in Pharmacy
- Current and unrestricted Pharmacist license in Texas
- 3+ years of experience as a licensed pharmacist
- 3+ years of patient consultation experience
- Beginner level proficiency in MS Office and Windows-based environments
