Key Responsibilities

• Provide ongoing support and resources to individuals living with dementia and their families as part of an interdisciplinary team
• Conduct virtual services including psychosocial assessments, therapeutic interventions, and crisis support
• Manage complex cases and collaborate with healthcare professionals to ensure holistic care delivery

Required Qualifications

• Active, unrestricted Texas LCSW license and residency in Texas
• 3+ years of experience in care management and behavioral health services related to dementia or geriatrics
• Experience providing virtual services is strongly preferred
• Knowledge of evidence-based practices for the geriatric population
• Ability to work collaboratively within an interdisciplinary team
