Texas Licensed Clinical Social Worker

Job Summary

To support a growing healthcare practice, the full-time Texas Licensed Clinical Social Worker will manage a caseload of high-risk patients, conduct psychosocial assessments, and coordinate care services in a hybrid work environment (90% remote, 10% in-field).

Key responsibilities

• Manage a caseload of high-risk socially complex patients
• Conduct face-to-face and telephonic psychosocial assessments with patients and caregivers
• Coordinate services to ensure understanding of treatment plans and develop shared goals of care

Required qualifications

• Master's degree in social work
• Must be licensed as a Texas LCSW
• Valid driver's license and good driving record
• Above average computer skills
• Experience in care management in a community-based setting is preferred
