Job Summary
To support patients and caregivers navigating complex dementia-related care needs, the full-time Licensed Clinical Social Worker (LCSW) will provide advanced care coordination, manage family dynamics, and facilitate psychosocial problem-solving in a remote environment.
Key Responsibilities
- Provide care coordination support for patients and caregivers with complex psychosocial needs
- Guide families through sensitive discussions and support caregivers in managing emotional responses
- Document interventions and ensure continuity of care for seamless transitions among team members
Required Qualifications
- Active Licensed Clinical Social Worker (LCSW) license in Texas
- Minimum of 3 years of experience in a clinical social work role, particularly with older adults and dementia
- Experience navigating complex family dynamics and psychosocial issues
- Strong communication skills and ability to provide support via phone and video calls
- Willingness to obtain additional state licenses as needed
