Job Summary
Collaborating with an interdisciplinary clinical team, the part-time Licensed Clinical Social Worker (LCSW) will deliver comprehensive, patient-centered palliative care remotely to individuals with serious illness, focusing on psychosocial support, care coordination, and advance care planning.
Key responsibilities
- Conduct comprehensive assessments to evaluate psychosocial distress, mental health needs, and social determinants of health
- Facilitate serious illness conversations and advance care planning with patients and families
- Provide clinical recommendations and support care coordination within the interdisciplinary team
Required qualifications
- Master's in Social Work (MSW) from an accredited program
- Active, unrestricted Licensed Clinical Social Worker (LCSW) license in Texas
- Clinical experience in palliative care, including advance care planning
- Experience working within an interdisciplinary care model
- Proficiency in using electronic health records (EHRs) and clinical documentation systems
