Job Summary

A company is looking for a Community Health Worker to provide support and outreach to members remotely in Texas.

Key Responsibilities

• Document member interactions and ensure compliance with established requirements
• Conduct outbound calls for appointment scheduling, provider outreach, and assess member needs
• Provide administrative support and inform members about available community resources

Required Qualifications

• High School Diploma/GED or 4+ years of medical customer service experience
• 2+ years of customer service experience, specifically in a medical setting
• Intermediate proficiency in Microsoft Office applications
• Access to high-speed internet and a designated quiet workspace at home
• Must reside in the state of Texas
