Texas Licensed Community Health Worker

Job Summary

A company is looking for a Community Health Worker to support Medicaid members in accessing care and managing their health.

Key Responsibilities:

• Create positive relationships with members and assist in goal setting and care plan development
• Engage members proactively to manage their health and connect them with necessary resources
• Coordinate care plans and collaborate with the care team to ensure quality services are provided

Required Qualifications:

• High School Diploma/GED (or higher)
• 3+ years of experience in a community health setting
• Intermediate proficiency in MS Word, Excel, and Outlook
• Access to a designated quiet workspace at home with secure handling of Protected Health Information (PHI)
• Must reside in Texas
