Job Summary

To support community health initiatives, the full-time Texas Licensed Community Health Worker will assess, plan, and implement individualized care strategies while working remotely, focusing on connecting members to social services and community resources.

Key responsibilities

• Create positive relationships with members and proactively engage them in managing their health
• Conduct post-discharge activities and connect members to necessary social services
• Advocate for patients and ensure quality coordinated care by communicating effectively with all stakeholders

Required qualifications

• High School Diploma/GED
• 3+ years of experience in a community health setting
• Intermediate computer proficiency with Microsoft Office and web applications
• Must reside in Texas and have a designated quiet workspace at home
• Access to high-speed internet service approved by UnitedHealth Group
