Job Summary
To support community health initiatives, the full-time Texas Licensed Community Health Worker will assess, plan, and implement individualized care strategies while working remotely, focusing on connecting members to social services and community resources.
Key responsibilities
- Create positive relationships with members and proactively engage them in managing their health
- Conduct post-discharge activities and connect members to necessary social services
- Advocate for patients and ensure quality coordinated care by communicating effectively with all stakeholders
Required qualifications
- High School Diploma/GED
- 3+ years of experience in a community health setting
- Intermediate computer proficiency with Microsoft Office and web applications
- Must reside in Texas and have a designated quiet workspace at home
- Access to high-speed internet service approved by UnitedHealth Group
